Keyword stuffing is a definite no no and is looked down upon by almost all the major search engines. Therese a fine line between having keyword rich valuable content and having paragraphs overflowing with keywords that aren't entirely relevant to the page's content.

White words used to be very common as a way to stuff your page with keywords without any visible text to the user so that only search engines would pick up the text. This only lasted so long before Google and many other engines started to remove websites from search results because it became considered cheating.

Originally it was just white text on a white background but when CSS became more popular people could type whole paragraphs of keywords and set the <div> to "Display None" this would make it entirely invisble to users but show up clear as day in the source code indexed by spiders.

For the most part, Yes.
Google, MSN, AOL, Ask and most big search engines are no longer using meta data to put together search results or for ranking purposes in Googles case.

However some smaller search engines still run off of a system relying almost entirely on meta data to tell the engine what the page is about, This makes having the meta tags a good idea even though its not going to get you anywhere with the big search engines. If your looking to move up in google focus on intelligent keyword usage ( avoid keyword stuffing ) and accurate page titles.
